AMEND THE IGA WITH VALLEYWISE FOR CAPITAL IMPROVEMENTS TO BEHAVIORAL HEALTH FACILITIES Amend the Intergovernmental Agreement (“IGA”) between Valleywise (“Contractor”) and Maricopa County (the “County”). The purpose of this amendment is to provide funding to Valleywise to make capital improvements to the Maryvale Hospital to create a second mental health courtroom and related space. Funding of this Agreement is through the American Rescue Plan Act, Coronavirus State and Local Fiscal Recovery Funds under the CDFA number of 21.027. AMENDMENT TO IGA WITH VALLEYWISE FOR CAPITAL IMPROVEMENTS TO BEHAVIORAL HEALTH FACILITIES Approve Amendment No. 3 to the financial Intergovernmental Agreement (“IGA”) between Maricopa County Special Health Care District (MCSHCD) dba Valleywise Health (“Subrecipient”) and Maricopa County (the “County”). The purpose of the Agreement is to respond to the public health emergency caused by COVID-19, and to provide funds to assist in the economic recovery from the pandemic, including programs and services for mental health treatment and behavioral health services. The County is authorized to distribute American Rescue Plan Act of 2021 (ARPA) funds to subrecipients to accomplish the purposes of ARPA under the CDFA/AL number of 21.027. The County provided the Subrecipient with $16 Million in ARPA funds to be spent on capital expenditures and improvements of behavioral health facilities, as outlined in the Agreement. The purpose of this amendment is to increase the amount of funds provided by Amendment No. 2 by $183,879, resulting in a new total not-to-exceed amount of $2,848,879 ($2,665,000 plus $183,879 = $2,848,879), to be used for Security and Storefront costs. AMENDMENT TO IGA WITH VALLEYWISE HEALTH Approve Amendment 4 to Intergovernmental Agreement (IGA) by and through the Department of Public Health and Maricopa County Special Health Care District (MCSHCD) dba Valleywise Health for court-ordered evaluations and treatment of persons with mental illness. I. The Parties agree to enter into this Amendment No. 4 to amend the Agreement as follows: A. The Agreement is hereby extended through June 30, 2025. II. Section I above contains all the changes made by this Amendment No. 4. All other terms and conditions of the Agreement and prior Amendments shall remain in full force and effect as executed by the Parties. III. The Parties have authorized the undersigned to execute this Amendment No. 4 on their behalf, and it shall be effective upon approval and signature by the Parties. (C-95-22-025-X-04)
