CONTRACT NO: PH RFP 220294 AMENDMENT NO: 1 AMENDMENT TO CONTRACT Between MARICOPA COUNTY by and through DEPARTMENT OF PUBLIC HEAL TH and West Valley OB-GYN I. The above-named contract is hereby amended as specified below: A. Section 1.0 CONTRACT TERM shall be updated to reflect a new expiration date of June 30, 2024. A new Timeline will go into effect beginning July 1, 2023 - June 30, 2024 and is shown on page 3 of this amendment. B. Exhibit A, PRICING - TOTAL REQUESTED shall be updated to $289,982. The increase of $144,991 are for services beginning in the new budget period starting July 1, 2023 and is shown on page 4 of this amendment. C. Section 6.0 TERMS AND CONDITIONS shall be updated to include the following additional clauses: i. 6.37 ii. 6.38 WRITTEN CERTIFICATION PURSUANT to A.RS.§ 35-393.01: If Contractor engages in for-profit activity and has 10 or more employees, and if this agreement has a value of $100,000 or more, Contractor certifies it is not currently engaged in, and agrees for the duration of this agreement to not engage in, a boycott of goods or services from Israel. This certification does not apply to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 U.S.C. § 4842. WRITTEN CERTIFICATION PURSUANT to A.RS. § 35-394: Contractor warrants and certifies that it does not currently, and agrees for the duration of the Agreement, that it will not use: 1. the forced labor of ethnic Uyghurs in the People's Republic of China, 2. any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China, 3. any contractors, subcontractors or suppliers that use the forced labor or any goods or services produced by the forced labor of ethnic Uyghurs in the People's Republic of China. If Contractor becomes aware during the term of this Agreement that the Contractor is not in compliance with this paragraph, Contractor shall notify the County within five business days after becoming aware of the noncompliance. If the Contractor fails to provide a written certification to the County that the Contractor has remedied the noncompliance within 180 days after notifying the County of its noncompliance, then the agreement terminates, except that if the agreement termination date occurs before the end of the 180-day period, the agreement terminates on the agreement termination date. Outreach and Communication Strategies • Launch an outreach campaign to raise awareness about the expanded testing services and resources available. • Utilize various communication channels such as social media, local advertisements, community events, and partnerships with community organizations. • Conduct targeted outreach to healthcare providers, community centers, schools, and other organizations to promote the initiative and facilitate referrals. Implementation of Expanded Testing Services • West Valley OB-GYN currently offers treatment and testing services Monday-Thursday, 9 AM-6 PM, and Friday, 9 AM-4 PM. Implementation of expanded testing services to include additional evening and weekend hours for STD/STI testing and treatment services at West Valley OB-GYN clinics. • West Valley OB-GYN offers walk-in clinic hours daily (Monday-Thursday 9 AM-6 PM, Friday, 9 AM-4 PM}, and onsite (4338 W. Thomas Road, 85031 ). Implementation of expanded testing to include additional onsite walk-in clinic hours and walk-in clinic sites at designated partner clinics and community events to provide convenient and stigma-free access to testing and treatment without appointments. • Coordinate with transportation services to assist patients in reaching the clinics, particularly those facing transportation barriers. Monitoring, Evaluation, and Continuous Improvement • Implement data collection mechanisms to monitor the utilization of services, patient outcomes, and barriers experienced. • Analyze data and evaluate the effectiveness of the initiative in reducing barriers and improving access to care. • Seek feedback from patients, community health workers, and healthcare providers to identify areas for improvement. • Make necessary adjustments based on the evaluation findings to optimize the initiative's impact and outcomes.
