SCOPE OF SERVICES — POST DESIGN (CHANGE ORDER # 08)

HDR will provide post design services during the construction of Gilbert Road Bridge at the Salt River
with anticipated construction duration of 24 months. The scope of services as outlined below reflects
additional post design services that were not included with Change Order (CO) # 06, dated 4/21/2020
for this project.

1100.1.3 Request for Information (RFIs)

HDR will provide official response to the RFIs issued throughout construction. Considering the
complex nature of the project coupled with 24-month construction duration, CO # 08 includes a total of
one hundred (100) RFIs in addition to the 22 RFIs that were included with CO # 06.

1100.1.4 Shop Drawing Submittals

HDR will review the following Structure and the Geotechnical shop drawings. The following shop
drawings are in addition to the shop drawings listed on CO #06. Hours included with this CO do not
include hours for shop drawings that have already been reviewed by HDR and by our subconsultants.

Structure Shop Drawings:

e Abutment foundation/shaft reinforcement

e Deck pour schedule/sequence

e Miscellaneous Metals-Expansion Joints etc

e Survey of top of girder elevations

e Girder bracing plan

e Additional miscellaneous structure submittals and resubmittals (max 6)

Geotechnical Shop Drawings:

e Drilled shaft integrity testing results (22 shafts)

e ADOT inspector reports and concrete placement logs

e CSA placement plan and mix design

e Additional miscellaneous geotechnical submittals (max 4)

1100.1.5 General Project Management

This task includes hours for project management and contract administration throughout the 24-month
duration for construction. CO # 08 includes an additional 60 hours for the PM and an additional 24
hours for project administration and coordination. CO # 06 included 12 hours for the PM and 24 hours
for project administration.

1100.1.6 Record Drawings

It is anticipated that MCDOT Survey team will prepare the Record Drawings once construction is
complete. HDR has not included any hours for the preparation of Record Drawings. Hours are included
for the coordination with MCDOT survey team.

1100.1.7 Design Modifications (MCDOT Requested)

The HDR team will address up to four (4) design modifications during construction. This work will only
be completed with a written request from MCDOT. HDR anticipates development of two (2) plan sheets
at 40 scale (half-size) for each design modification. Hours have been included for the development of
the design modification and plan sheets including QC and coordination with MCDOT.
