required to make an informed recommendation.

Due to Mr. Farhi’s failure to provide the required City forms and information in a
timely manner the Police Department was unable to conduct a thorough review of
the application. Therefore, the Police Department is recommending disapproval of
this liquor license application because he has not shown that he is capable, qualified
or responsible to hold a liquor license in the City of Phoenix.
