Re: County License No. P50275 – Public Health Department 
 
Maricopa County (County) and Phoenix Day (Contractor) are parties to that certain Facilities Use/Revocable License 
Agreement dated October 15, 2015, as amended (Agreement) regarding the premises located at 8828 North Central 
Avenue, Phoenix, AZ, and 5422 West Thunderbird Road, #6, Glendale, AZ. The Agreement continues until terminated by 
either party. Pursuant to Section 2 of the Second Amendment to the Agreement, County hereby provides Contractor with 
thirty (30) days’ prior written notice of termination of the Agreement on June 30, 2023.
