CHANGE ORDER # 4 
 
THIS CHANGE ORDER # 4 
 is entered into as of ________________ 
between the City of Buckeye, an Arizona municipal corporation 
Municipal 
Emergency Services Inc., a Nevada corporation 
. This Change Order modifies 
certain sections of Contract No. 2025158 
, with those sections not expressly 
modified remaining in full effect pursuant to their terms.  
 
RECITALS: 
 
The City and Contractor entered into Contract No. 2025158 dated August 19, 2024. Pursuant to 
which Contractor was to provide firefighting equipment and rescue tools with related supplies and 
accessories with an expiration date of April 4, 2028 and a not-to-exceed amount of $225,000.00. 
 
Thereafter, Change Order #1 to change the not-to-exceed amount by $500,000.00 from 
$225,000.00 to $725,000.00 was entered into as of December 17, 2024.  
 
Thereafter, Change Order #2 New Section, Trade-In and Buy-Back Credits Authorized.  
Contractor may offer the City credit for any City personal property towards the purchase of any 
goods or services under this agreement was entered into as of May 12, 2025. 
 
Thereafter, Change Order #3 to change the not-to-exceed amount by $600,000.00 from 
$725,000.00 to $1,325,000.00 was entered into as of January 13, 2026. 
 
 
CONTRACT CHANGES: 
 
1. 
Contract Price, is amended by increasing the not-to-exceed amount by 
$4,675,000.00 from $1,325,000.00 to $6,000,000.00. 
 
 
ADDITIONAL PROVISIONS: 
 
Except as amended herein, all provisions of the Contract remain in full force and effect. 
 
In the event of any conflict or discrepancy between the terms of this Change Order, the 
Contract, and any other contract documents, the documents shall govern in that order.

This Change Order may be canceled pursuant to the provisions of A.R.S. § 38-511.
