Project Narrative: (provide additional sheets as needed)

214 S 5th Street is a facility that started out as a church built in 1916 It later had an addition put on to include additional class rooms, multi-purpose use with a kitchen area
and then finally in the mid 1990's a fellowship hall This facility has been a community outreach program providing basic resources and spiritual care to the community
around it for the last 98 years The facility came Into the possession of Ail Faith Community Services, a non-profit 501(c)3 organization in 2043 to provide continuation
and enhancement of services to the city of Buckeye All Faith has a 99 year lease with Skyway Church of the West Valley for $1 a year to provide to the community basic
resources All Faith ts the contracted food bank through St Mary's Food Bank Alliance

The facility is in need of upgrades to make It more energy efficient and and create a more secure environment for those of the community using the services

In the area surrounding the facility are many residential homes that have or are going through renovation and Increasing the street view and property values to the homes
All Faith's desire Is to be a community partner and compliment or even lead the neighborhood in developing a desirable look that others would be happy to live beside, but

also to those who may question the safety and well being of their coming to the area to visit or even to volunteer at our facility

Description of Work: (provide additional sheets as needed)
The scope of work for this project would include the replacement of 14 windows 12 windows of which are still old single pane windows that are puttied and pinned tn
These 12 windows would be replaced with mill guard brand with maximum energy efficiency ~The 13th window will replace a more recent window but will be put in to
match the 12 being replaced and also to be maximum energy efficient The 14th Window replaced will be a service window that will allow for maximum efficiency as it Is

a west facing window and will create a more secure and effective source of service to the community it helps

Additional work will include the replacement of old wood framed in metal core doors that lack closures and push bars for safety and energy efficiency 6 new metal frame

doors will be installed with closures, emergency exit bars and new thresholds and weather sealed

Security lighting is another element of this project Current exterior lighting is lacking in areas where there are dark corners All Faith regularly encounters those who look
to hide and or sleep in dark corners of our facility along with issues of theft of items Additional photo sensor led flood lighting will provide a stronger security presence
and discourage those coming outside of business hours

Finally, the structure is currently block with skim coat of stucco that is cracking and peeling and in need of a fresh clean look The proposal ts to install a manufactured stone
siding along the bottom 1/3rd of the front exterior coming all the way up with stone work on the face of the main entrance Metal siding will be used to wrap existing wooden
framework exposed and metal facia to finish off the look This stone work will also wrap around the frames of the newly installed windows Fresh paint will coat the

remaining exposed areas

The Basis of Estimate is a written explanation clarifying the scope, assumptions and exclusions used in establishing
the estimate dated March 31%, 2025.

All costs are based on CORE's incorporation of the scope described during the site walk held on 3/24/2025 and the
scope clarifications below. This pricing is based on the assumption that any cost above and beyond what CORE has
assumed will be addressed via contingency or owner change order, and if the funding 1s not available, these items
will be addressed via value engineering to do our best to perform as much scope as possible within the budget that
has been allocated.

ASSUMPTIONS, CLARIFICATIONS, & EXCLUSIONS

Contingency

¢ Construction Contingency of 2 5% ($2,899.00 direct cost) is included in this proposal — Construction Contingency
is intended to be used at CORE’s discretion to cover costs that have not been identified as a trade specific scope
on the Schedule of Values setting documents and may require further clarification or coordination. These costs
may include scope gap, coordination issues between trades, and missed scope during the subcontractor bidding
process. Construction Contingency does not account for design revisions or additional scope requests made
by the Owner or Architect.

¢ Owner/Design Contingency — is NOT included in this proposal.

¢ Escalation Contingency ~ is NOT included tn this proposal.

General Assumptions

¢ This proposal is valid for 15 days.

e Any cost savings from approved value engineering after executed proposal will be added to the Construction
Contingency.

¢ CORE has not included any costs for building permit, mechanical/electrical/plumbing permits. We have
included a Dust permit and NESHAP for the demo.

e We assume normal working hours.

¢ This proposal includes all costs related to warranty as required in the contract.

« Any increased costs associated with tariffs promulgated after the contract execution date are not included. The
contractor will assist the owner with sourcing alternative materials not affected by the tariff but in the event that
the owner chooses to retain the originally selected material or there is not another material option available
that does not affect the original budget or project schedule then the contractor ts entitled to a change order
for the additional cost and time associated with the enactment of the applicable tariffs.

¢ This proposal includes all costs related to warranty as required in the contract.
