April 12, 2023 C-26-19-023-3-03 AMENDMENT No. 3 To MEMORANDUM OF UNDERSTANDING Between HEALTH CHOICE AZ, INC & MARICOPA COUNTY, ARIZONA WHEREAS, Maricopa County (“County”) and Health Choice AZ, Inc. (“Contractor”) have entered into an Agreement for the purpose of establishing collaborative protocol executed June, 18th, 2019 (“Agreement”). WHEREAS, County and Contractor have agreed to renew this Agreement. NOW, THEREFORE, the parties hereby agree to the following provisions: 1. The Agreement shall be renewed for a term from May 17th, 2023 and ending May 16th, 2024. ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall control. This Amendment is subject to cancellation pursuant to A.R.S. § 38-511.
