THIS CHANGE ORDER # 1 
 is entered into as of _____________ between 
the City of Buckeye, an Arizona municipal corporation 
 Vincere Physicians Group 
PLLC, an Arizona professional limited liability company 
. This Change Order 
modifies certain sections of Contract No. 32500033 
, with those sections not 
expressly modified remaining in full effect pursuant to their terms.  
 
RECITALS: 
 
The City and Contractor entered into Contract No. 32500033 dated March 17, 2025. Pursuant to 
which Contractor was to provide cancer screening for firefighters with an expiration date subject 
to the terms of the City of Tempe contract T23-039-01 and a not-to-exceed amount of $160,000.00. 
 
CONTRACT CHANGES: 
 
 
1. 
Contract Price, is amended by increasing the not-to-exceed amount by $180,000.00 
from $160,000.00 to $340,000.00. 
 
2. 
This Change Order increase is intended to cover expenses associated with cancer 
screenings through the life of the contract with an expiration date of October 20, 2026.  
 
 
 
ADDITIONAL PROVISIONS: 
 
Except as amended herein, all provisions of the Contract remain in full force and effect. 
 
In the event of any conflict or discrepancy between the terms of this Change Order, the 
Contract, and any other contract documents, the documents shall govern in that order.   
 
This Change Order may be canceled pursuant to the provisions of A.R.S. § 38-511.
