DBA Construction, Inc., an Arizona corporation 
). This Change 
Order modifies certain sections of Contract No. 2025263 
, with those sections not 
expressly modified remaining in full effect pursuant to their terms.  
RECITALS: 
The City and Contractor entered into Contract No. 2025263 dated January 7, 2025. Pursuant to 
which Contractor was to provide construction services for the Rooks Road Lateral 18 project, 
which remains in effect for the remainder of the City of Peoria
 and a not-to-
exceed amount of $1,599,707.23. 
CONTRACT CHANGES: 
1.
Exhibit B of the Original Contract  Proposal, is amended to incorporate the
additional services set forth in Exhibit 1 of Change Order #1. The Change Order adds the water 
main extension construction services for the Rooks Road project. 
2.
Contract Price, is amended by increasing the total not-to-exceed amount by
$1,089,172.00, from $1,599,707.23 to $2,688,879.23. 
ADDITIONAL PROVISIONS: 
Except as amended herein, all provisions of the Contract remain in full force and effect. 
In the event of any conflict or discrepancy between the terms of this Change Order, the 
Contract, and any other contract documents, the documents shall govern in that order.   
This Change Order may be canceled pursuant to the provisions of A.R.S. § 38-511.
