\, Vim D oty , do hereby state under oath the following:

1. lam the NAO) Pesca for Love’s Travel Stops & Country Stores, Inc., an Oklahoma limited liability

company (the “Company”).

2. The Company is a domestic Oklahoma corporation and is registered to do business in Arizona as a foreign
corporation.

3. On January 13, 1997, the Company filed an Amended and Restated Certificate of Incorporation with the
Oklahoma Secretary of State changing the name of the Company to “Love’s Country Stores, Inc.”

4. | The Company purchased real property in Buckeye, AZ through a Warranty Deed recorded on January 26, 2000,
at instrument number 00-0062609 under the name “Love’s Country Stores”.

5. On November 15, 2000, the Company filed an Amended Certificate of Incorporation with the Oklahoma
Secretary of State changing the name of the Company to “Love’s Travel Stops & Country Stores, Inc.”

6. The name of the Company is currently Love’s Travel Stops & Country Stores, Inc. and the name of the Company
has not been changed since November 15, 2000.

7. The entity that was registered under the name “Love’s Country Stores, Inc.” is the same entity currently

operating as Love’s Travel Stops & Country Stores, Inc., and therefore, the entity that purchased the property
described above is the same as the purchasing entity and is now known as Love’s Travel Stops & Country Stores,

Inc.
