COOPERATIVE CONTRACT PURCHASE ADDENDUM (CCPA)  
BETWEEN 
THE CITY OF BUCKEYE 
AND 
BALZIC ENTERPRISE, LLC DBA M&J TROPHIES AND APPAREL  
CONTRACT NO. 2025066 
CHANGE ORDER # 2 
THIS CHANGE ORDER # 2 (this “Change Order”) is entered into as of _______________
between the City of Buckeye, an Arizona municipal corporation (the “City”) and Balzic Enterprise, 
LLC dba M&J Trophies and Apparel, an Arizona limited liability company (the “Contractor”). 
This Change Order modifies certain sections of Contract No. 2025066 (the “Contract”), with those 
sections not expressly modified remaining in full effect pursuant to their terms.  
 
RECITALS: 
 
The City and Contractor entered into Contract No. 2025066 dated July 12, 2024. Pursuant to which 
Contractor was to provide general printing services with an expiration date pursuant to City of 
Peoria contract ACON09723 and a not-to-exceed amount of $120,000.00. 
 
Thereafter, Change Order # 1 to change the not-to-exceed amount by $75,000.00 from 
$120,000.00 to $195,000.00 was entered into as of August 7, 2024.  
 
 
CONTRACT CHANGES: 
1. 
Contract Price, is amended by increasing the not-to-exceed amount by $605,000.00 
from $195,000.00 to $800,000.00. 
 
2. 
No Boycott of Israel.  To the extent applicable under A.R.S. § 35-393 through § 35-
393.03, the Contractor hereby certifies that it is not currently engaged in and agrees for the duration 
of this Agreement to not engage in, a “boycott” of goods or services from Israel, as that term is 
defined in A.R.S. § 35-393(1). 
 
ADDITIONAL PROVISIONS: 
 
Except as amended herein, all provisions of the Contract remain in full force and effect. 
 
In the event of any conflict or discrepancy between the terms of this Change Order, the 
Contract, and any other contract documents, the documents shall govern in that order.   
 
This Change Order may be canceled pursuant to the provisions of A.R.S. § 38-511.
