THIS CHANGE ORDER #1 
 is entered into as of _____________________, 
between the City of Buckeye, an Arizona municipal corporation 
Shums Coda 
Associates, Inc., a California corporation (the 
). This Change Order modifies certain 
sections of Contract No. 32600026 
Agreement , with those sections not expressly modified 
remaining in full effect pursuant to their terms.  
 
RECITALS: 
 
The City and Consultant entered into Contract No. 32600026 dated July 24, 2025. Pursuant to 
which Consultant was to provide Plan Check Review, Inspection, and Training Services with an 
expiration date of July 23, 2027, and a not-to-exceed amount of $100,000.00. 
 
CONTRACT CHANGES: 
 
1. 
Section 2  Scope of Work and Exhibit A of the Original Agreement  Scope of 
Work and Fee Proposal, are amended to incorporate the additional services set forth in Exhibit 1 
of Change Order #1. 
 
2. 
Section 3  Compensation, is amended by increasing the not-to-exceed amount by 
$400,000.00, from $100,000.00 to $500,000.00.  
 
ADDITIONAL PROVISIONS: 
 
Except as amended herein, all provisions of the Contract remain in full force and effect. 
 
In the event of any conflict or discrepancy between the terms of this Change Order, the 
Agreement, and any other contract documents, the documents shall govern in that order.   
 
This Change Order may be canceled pursuant to the provisions of A.R.S. § 38-511.

For over the past couple of years Shums Coda Associates (SCA) has enjoyed working with your staff 
to provide building life-safety services. Using a third-party contractor, such as SCA, allows flexibility to 
offer superb customer satisfaction during times of construction ebbs and flows.  
Our hourly rates are as follows:
