This applies to your project titled “Buckeye Virtual Reality Learning and Exploration Program,” which has been approved by the Arizona State Library for full funding in the amount of $8000. Your submitted application, the attached official notification and certifications, and the criteria outlined in this email will be the only contractual documents necessary for the implementation of your project. Federal funds can only be used as stipulated in the grant guidelines found on the State Library’s LSTA webpage and the attachments to this email. Projects may be monitored periodically by State Library staff. Please ask your Legal Administrator to print out, sign, and return the following attached documents: General Assurances and Certifications Internet Safety Certification (for public and school libraries only) Non-Construction Assurances Request for Payment Form All signed documents should be emailed to libdev@azsos.libanswers.com. 2 Please note: As stated in the Award Notification document, "this award is subject to the availability of funding from the Institute of Museum and Library Services (IMLS)". Because of current uncertainty around federal funding, we are asking that all awardees submit their requests for payment with their signed forms in advance rather than spending money on the project that the State Library would be unable to reimburse if federal funds become unavailable. The Arizona State Library has been officially awarded LSTA 2025 funds from IMLS that we may request to draw down incrementally. The sooner awardees request funds from us, the sooner we can request them from IMLS and disburse to awardees. Also, we will wait until all funding has been disbursed to recipients before sharing publicly the list of grant awards. We suggest that you also wait until you have received your funds before announcing your grant award. You will find downloadable files to support your project's management in this Google Drive folder. This includes the LSTA 2025 Grant Recipients Information Packet, survey forms (required for programs targeting adults), and some promotional template materials. Notice for New Recipients and Those with Organizational Updates: A W-9 and an Automated Clearing House (ACH) form are also available in the Google Drive folder under the LSTA Finance Forms subfolder. Please complete and return the W-9 if there have been changes to your organization. If the W-9 on file with the State is out-of-date, the funds may be disbursed incorrectly. We encourage all vendors to sign up to receive payments via direct deposit to expedite payments and to reduce the risk that the checks are lost in the mail. Please email the signed ACH form to vendor.payautomation@azdoa.gov. After the form is submitted, the AZ General Accounting Office will call you to confirm the ACH setup. As mentioned in the initial notification email, all recipients of LSTA grants are required to either attend (general grant recipients) or watch the recording (express grant recipients) of the Grant Recipients’ Workshop. For this year, it will be held virtually only, on June 23, 2025, from 9:00 AM - 10:00 AM.
