Contract No. C-22-22-117-X-01 Amendment No. 1 Scottsdale Contract No. 2022-094-COS-A1 Page 1 of 5 AMENDMENT NO. 1 TO THE INTERGOVERNMENTAL AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND CITY OF SCOTTSDALE I. Maricopa County (“County”), administered by its Human Services Department, and the City of Scottsdale, (“Contractor”) entered into a financial Intergovernmental Agreement (“Agreement”), which was fully executed on or about July 27, 2022. The purpose of the Agreement is for the Contractor to provide Community Action Program (CAP) services to include Crisis Case Management, coordination of services to assist low-income households in crisis situations, and assistance to move closer to economic self-sufficiency. The Contractor shall provide services in Scottsdale and surrounding areas. The Agreement term is the term of July 1, 2022 through June 30, 2023. The County provided the Contractor with $391,875 for the CAP service activities. The County and the Contractor collectively are referred to as the “Parties.” II. The Parties agree to enter into this Amendment No. 1 to amend the Agreement as follows: A. Revise the Contract Amount reflected on page 1 of the Agreement to reflect a total contract amount of $453,750. B. Revise Section 4 (Budget and Compensation), paragraph 1.2 (Funding), subparagraph 1.2.3 by removing in its entirety and replacing with the following: 1.2.3 Funding shall be provided in six (6) month timeframes to support minimum staffing levels as stated below: 1.2.3.1 Funding for the timeframe of July 1, 2022, through December 31, 2022, will support a minimum staffing level of: 1.2.3.1.1 5.5 full-time equivalent (FTE) positions to include 4.5 Client Services Specialists and 1 Quality Assurance Specialist. 1.2.3.2 Funding for the timeframe of January 1, 2023, through June 30, 2023, will support a minimum staffing level of: 1.2.3.2.1 5.5 full-time equivalent (FTE) positions to include 4.5 Client Services Specialists and 1 Quality Assurance Specialist. C. Revise Section 4 (Budget and Compensation), paragraph 1.5 (Operating Budget) by removing in its entirety and replacing with the following Operating Budgets. The County shall provide the Contractor with increased funding in the amount of $226,875 for the period of January 1, 2023, through June 30, 2023. The Agreement amount is now increased to a total of $453,750. Section II above contains all the changes made by this Amendment No. 1. All other terms and conditions of the Agreement remain in full force and effect as amended by previous amendments. IV. The Parties have authorized the undersigned to execute this Amendment No. 1 on their behalf. V. This Amendment No. 1 shall be effective upon approval and signature by both Parties.
