The above-named contract is hereby amended as specified below:
A. Exhibit A, Pricing TOTAL BUDGET ALLOCATION is changed to $352,030. The
additional $11,700 is for the additional services beginning on page 2 of this
amendment.
II.
All other terms and conditions of the original contract shall remain in full force and
effect.

This service order is to amend the Agreement between Maricopa County Department of Public
Health and Providertech to revise the post visit survey associated with the Health Literacy grant.
•
• Update survey logic in Providertech platform for patients with a completed visit
• Surveys will be sent to all patients who were identified as eligible and scheduled as part
of the Health Literacy grant outreach project o This includes the patients from the 3 FQHCs
associated with the grant
• Survey will be sent via text message with an embedded URL
• Survey results can be viewed and exported via the Providertech administrative portal.

This service order is to amend the Agreement between Maricopa County Department of Public Health
and Providertech to complete a secondary cultural and linguistically appropriate (CLAS) evaluation of
revised message content by the Southwest Interdisciplinary Research Center (SIRC) research partner
associated with the Health Literacy grant to ensure message content meets or exceeds CLAS standards.
• Providertech to review and share the project message content associated with project outreach
to SIRC for CLAS evaluation. This includes review of: o initial outbound messaging from Providertech
content library
o message content that was revised by FQHC partners
o suggested response handling content
o messaging in English and Spanish
• Providertech to review SIRC feedback and implement suggestions to existing content
• Revised content will be added to the Providertech administrative portal for ongoing patient
engagement
Description of one-time costs
• Project management o Amass and submit content to SIRC research partner
• Review feedback from SIRC
• Revise and update content in Providertech platform based on SIRC feedback
