EMERGENCY MEDICAL TRANSPORTATION SERVICE AGREEMENT EXTENSION
THIS EXTENSION OF THE CITY OF EL MIRAGE EMERGENCY MEDICAL TRANSPORTATION AGREEMENT,
approved by the Arizona Department of Health Services by letter dated September 20, 2023, (“Contract”), is 
entered into this 19th day of September 2023 by and between the City of El Mirage, an Arizona municipal 
corporation (the "City") and American Medical Response of Maricopa LLC (CON 136) ("AMR") (hereinafter 
individually "Party" and collectively "Parties").
RECITALS
A   The City and AMR entered into the City of El Mirage Emergency Medical Transportation Service 
Agreement effective September 20, 2023 ("Contract") with the initial Term of the Contract to 
expire September 1 9 , 2026.
B.
The Contract pursuant to Section 2(b); Term, Extension and Agreement Modification allows for up to three 
(3) additional one (1) year extensions upon agreement of the Parties.
C.
The Parties seek to exercise the available extension to extend the Term as per m it ted by 
Section 2(b) of the Contract. 
AGREEMENT 
NOW, THEREFORE, the Parties agree as follows:
1.
Recitals
 
 . The Recitals set forth above are not merely recitals, but are acknowledged to be true and 
correct, form an integral part of the Amendment, and are incorporated herein by this reference.
2.
Term
 
 . The term of the Contract is extended for the period from midnight on September 20, 2026, to 
midnight on September 19, 2027, unless superseded by a new contract or terminated or canceled as 
otherwise set forth in the Contract.
3.
All of the terms and conditions of the Contract and any Amendments thereto, which are not 
expressly modified or amended by this Extension, shall remain in full force and effect. This 
Extension constitutes the entire agreement between the Parties with respect to amending the Term 
of the Contract and supersedes any prior agreement, understanding, negotiation, draft agreements, 
discussion outlines, correspondence and memoranda or representation regarding amending the Term 
of the Contract.
IN WITNESS WHEREOF, the Parties have entered into this Extension effective on the date first written 
above and shall become effective upon approval and execution by both parties and approval of the Arizona 
Department of Health Services.
