PETITION FOR THE ESTABLISHMENT OF A
STREET LIGHTING IMPROVEMENT DISTRICT

TO THE BOARD OF SUPERVISORS OF MARICOPA COUNTY, ARIZONA:

We, the undersigned, being a majority of the persons owning real property within the
limits of the area hereinafter described, do hereby petition that the Board of Supervisors of
Maricopa County, Arizona, establish a street lighting improvement district, pursuant to the
provisions of Title 48, Chapter 6, Article 1, A.R.S., 1956, as amended and specifically
Sections 48-960 and 48-961, for the purpose of permitting the improvement of such area by
the establishment of street lighting facilities and to permit the improvement district to
purchase electric service for the lighting of such public streets and parks within the area
described.

It is requested that the Board of Supervisors establish the boundaries and declare the
street lighting improvement district organized under the corporate name hereinafter
proposed, and that such order contain such other matters and items as may be necessary to
authorize such street lighting improvement district to fully accomplish the purpose for which
it is formed.

The name of the proposed street lighting improvement district shall be

It is necessary that the proposed street lighting improvement district be organized because
the area hereinabove described is wholly outside the boundaries of any incorporated town or city
and it is impossible for the residents and property owners of the proposed district to secure the
lighting for streets and parks within the area described.

The public convenience, necessity and welfare will be promoted by the establishment of
the proposed street lighting improvement district and the property therein contained will be
benefited.

The boundaries of the proposed district are described as follows:

It is proposed that the street lighting facilities be installed at no cost to the street lighting
improvement district and that upon the installation and the execution of a contract between the
Arizona Public Service Company and this proposed street lighting improvement district, which
identifies the type of lighting for said facilities, that electric service be purchased for the
lighting of the public streets and parks for said proposed district.

It is further requested that upon the organization of the proposed street lighting
improvement district that the Board of Directors thereof appoint the Streets Superintendent as
district engineer and thereafter by resolution the board approve the type of lighting and rate for
this proposed district as presented by the Street Superintendent, and that the board declare its
intention to authorize an agreement with Arizona Public Services Company for the purpose of
purchasing energy for the use in lighting the public streets and parks withing the described
boundaries.

We petition that, as a property owner(s) within the above-mentioned proposed improvement
district, the County Board of Supervisors take necessary action to establish this street lighting
improvement district. I/We am/are aware that the cost of purchasing energy for the use in
lighting the streets and parks will be levied upon the property of the district and collected as
general county taxes on the annual property tax bill.
