AMENDMENT TO CITY OF CHANDLER AGREEMENT
LONG-TERM DISABILITY INSURANCE FOR PUBLIC SAFETY PERSONNEL
CITY OF CHANDLER AGREEMENT NO. 4046
THIS CALENDAR YEAR 2027 AMENDMENT (CY 2027 Amendment) to Agreement No. 4046 for Long-
Term Disability Insurance for Public Safety Personnel, dated January 1, 2004 (the Agreement),
is made and entered by and between the City of Chandler, an Arizona municipal corporation (City), and
Standard Insurance Company (Contractor) (City and Contractor may individually be referred to as Party
and collectively referred to as Parties) and made , 2026 (Effective Date).
RECITALS
WHEREAS, the City first entered the Agreement with Anthem Life, the predecessor-in-interest
of Contractor, to provide Long-Term Disability Insurance for Public Safety Personnel effective
January 1, 2004; and
WHEREAS, in 2025, the Agreement was amended to formally recognize Contractor as the
successor-in-interest to Anthem Life’s rights, interests, and obligations under the Agreement and
extend the Agreement for one year at a rate guaranteed through Calendar Year 2026; and
WHEREAS, Contractor offered to extend the guaranteed rate through December 31, 2028; and
WHEREAS, Contractor is in the process of transitioning from its predecessor’s policies and will
issue a new policy number and new policy documents effective May 1, 2027, with no change to
the City’s rates, rate guarantee, or plan design; and
WHEREAS, execution of a new agreement will be required to incorporate the Contractor’s new
policy documents for the period of May 1, 2027, through the end of the rate guarantee period on
December 31, 2028; and
WHEREAS, in anticipation of this change, the Parties wish to extend the Agreement under current
policy number AL00001212 at the guaranteed rate and subject to the same terms and conditions
for the period of January 1, 2027, through April 30, 2027.

AGREEMENT
NOW THEREFORE, the Parties agree as follows:
1. The recitals are accurate in every respect and are incorporated and made a part of the parties’
Agreement and the Amendment by this reference.
2. The Agreement is hereby extended at the guaranteed rate, $0.36/$100, for the period of January
1, 2027, through April 30, 2027.
3. All other terms and conditions of the Agreement remain unchanged and in full force and effect.
If a conflict or ambiguity arises between this CY 2027 Amendment and the Agreement, as
amended, the terms and conditions in this CY 2027 Amendment prevail and control.
IN WITNESS WHEREOF, the Parties have entered into this Amendment on the Effective Date.
