Docusign Envelope ID: 3FC5D64B-DFAA-8F4C-8142-B7D3D3307F4B City Clerk Document No. City Council Meeting Date: October 15, 2026 AMENDMENT TO CITY OF CHANDLER AGREEMENT DENTAL SERVICE PLAN CITY OF CHANDLER AGREEMENT NO. 1193 THIS CALENDAR YEAR 2027 AMENDMENT (CY 2027 Amendment) to Agreement No. 1193 for a dental service plan (the Agreement), is made and entered by and between the City of Chandler, an Arizona municipal corporation (City), and Delta Dental Plan of Arizona, Inc. (Contractor), (City and Contractor may individually be referred to as Party and collectively referred to as Parties) and made , 2026 (Effective Date). RECITALS WHEREAS, the Parties entered into an Agreement to provide Employer Group Dental benefits effective January 1, 2002; and WHEREAS, the Agreement has been annually amended to adjust the plan and rates and extend the term of the Agreement through December 31, 2026; and WHEREAS, Contractor proposed guaranteed renewal rates for the two-year period from January 1, 2027, through December 31, 2028; and WHEREAS, the Parties wish to extend the Agreement at the guaranteed renewal rates and subject to the same terms and conditions for an additional year through December 31, 2027. AGREEMENT NOW THEREFORE, the Parties agree as follows: 1. The recitals are accurate and are incorporated and made a part of the Agreement by this reference. 2. The Agreement is amended to replace the current rates with the renewal rates set forth in Exhibit A, attached hereto and incorporated herein by reference. The term of the Agreement is extended for one year from January 1, 2027, through December 31, 2027, subject to the same terms and conditions. 4. All other terms and conditions of the Agreement remain unchanged and in full force and effect. If a conflict or ambiguity arises between this CY 2027 Amendment and the Agreement, the terms and conditions in this CY 2027 Amendment prevail and control. Participation requirements: 100% Rating Assumptions The fee is valid for the effective date noted above and is guaranteed through the period shown above. Fee includes broker commission, if applicable. A directory of participating dentists is available online at www.deltadentalaz.com/find Group will provide each Subscriber with copies of the Certificate, the applicable Summary(ies) of Benefits, the Appeals Packet (if applicable), and all privacy notices and other notices from Delta Dental as may be required by any applicable federal or state law, at such intervals as may be required by law.
