AMENDMENT NO. 2 TO CONTRACT BETWEEN MARICOPA COUNTY BY AND THROUGH
THE DEPARTMENT OF PUBLIC HEALTH AND CITY OF CHANDLER, ARIZONA FOR HEAT
RELIEF SERVICES
I. The Parties to the above-named Agreement hereby agree to enter into this
Amendment No. 2 as specified below:
A. This revision includes transfers exceeding ten percent (10%) between
funded line items and therefore requires an amendment pursuant to
Section III. 6. Budget Schedule, Subparagraph 6.2.
B. Revise Section III. 6. Budget Schedule by updating line items as seen in
Exhibit A.
II. All other terms and conditions of the original contract shall remain in full force
and effect.
