The above-named contract is hereby amended as specified below: A. Maricopa County will exercise the automatic renewal clause as stated in Section V, Term and Default, paragraph B, extending the contract expiration date through October 31, 2027. B. Exhibit D, Fees for Nurse-Family Partnership Services updated 2026 and 2027 Implementing Partner Organization Fees shall be added for the new term as shown on page 2 of this amendment. C. An additional $60,000 for the new term will be added, bringing the not-to-exceed (NTE) total to $300,000. II. All other terms and conditions of the original contract shall remain in full force and effect.
