CONSULTANT’S SCOPE OF WORK
ON CALL SUBSURFACE UTILITY ENGINEERING DESIGNATION &
POTHOLE SERVICES
1. GENERAL
1.1 Definition and Terms
• CONSULTANT: The individual or firm directly providing
utility designating and locating services as a party to the
contract.
• MUTCD: "Manual on Uniform Traffic Control Devices", U.S.
Department of Transportation.
• QL A: Indicates the precise horizontal and vertical location
of utilities obtained by the actual exposure and subsequent
measurement of subsurface utilities, typically at a specific
point. Accuracy is typically 15-mm vertical and horizontal.
• QL B: Indicates information obtained through the application
of appropriate surface geophysical methods to determine the
existence and approximate horizontal position of subsurface
utilities and should be reproducible by surface geophysics at
any point of their depiction.
• QL C: Indicates information obtained by surveying and
plotting visible above ground utility features and by using
professional judgment in correlating such information to QL
D information.
• QL D: Indicates information derived from existing records
and oral recollections.
• MCDOT: Maricopa County Department of Transportation
• SUE: Subsurface Utility Engineering, a branch of
engineering practice that manages risks associated with
underground infrastructure by locating, mapping, and
protecting utilities using vacuum excavation and geophysics
technology.
• Designate: The term “designate” means both to indicate, by marking, the
presence and horizontal location of a subsurface utility using geophysical
prospecting techniques and mapping as well as mapping above ground utilities.
The CONSULTANT shall use its best efforts and expertise in designating all
requested surface utilities and visual observation for above ground utilities.
• Locating: The term “locating” means to obtain the accurate
horizontal and vertical location of a subsurface utility by
digging a pothole as described in this document.
• Mobilization: All mobilization costs for designating and

1.2 CONSULTANT and its subcontractor’s or subconsultants and their
respective employees, agents, and representatives, when
performing the work described in Exhibit A, Scope of Work, shall
comply with all rules, regulations and mandates set forth by the
COUNTY, State or Federal governments or any other entity having
jurisdiction and shall adhere to all laws and regulations pertaining
to occupational health, environmental protection and all other
regulations and ordinances which apply to any work performed
pursuant to the contract.
1.3 The intent of this contract is to achieve accurate utility inventories,
through the application of SUE techniques. The primary services
anticipated to be rendered hereunder are QL A and QL B
mapping.
1.4 The CONSULTANT will assist MCDOT in developing the scope of
work for a project by assessing project SUE needs, generating
alternatives, and/or making recommendations. The
CONSULTANT will prepare plans showing the existing utility
facilities on CADD using utility companies’ installation records and
verification by surveillance techniques employed by the
CONSULTANT. The CONSULTANT will excavate potholes using
the vacuum extraction method or other noninvasive means to
accurately locate underground utility both horizontally and
vertically and to record this information on CADD.
1.5 Two types of utility location may be utilized. Type 1 consists of
“designating” services (horizontal mapping of existing utility
facilities) and Type 2 consists of “subsurface utility locating”
(locating {pothole}) services. The CONSULTANT provides this
information for use by MCDOT’s design engineering team to
assure that utility conflicts at the time of construction are
eliminated. The Consultant will provide all equipment, personnel
and supplies for this service that shall be performed. The
Consultant shall make reasonable provision for MCDOT
representatives to observe and inspect the Consultant’s work in
process.
1.6 Written Notice to Proceed (NTP) will be given for each project for
designating and again for the locating (pothole) prior to any work
being performed. Some projects may combine both designating
and potholing to meet the project needs; or as determined by the
MCDOT Utility Project Manager. MCDOT shall not be responsible
for payment for work done in advance of such notice. Each task
order will contain an expected due date for the completion of the
work. If at any time during the work the CONSULTANT feels he
needs more time to complete the work the CONSULTANT is to
notify the MCDOT Utility Project Manager of the reason for the
delay along with providing a new date for delivering the data and
mapping. Failure to provide the required documentation to
MCDOT will delay payment of services to the CONSULTANT until
the matter is corrected.

during all phases of excavation, beginning with CONSULTANT’S
mobilization of equipment and personnel until all work has been
completed and the site has been restored to its original state.
CONSULTANT’S responsibility for the site during the period
specified above shall not be limited to CONSULTANT’S working
hours and shall include but not necessarily be limited to the
following:
• Traffic Control for short-term shoulder work only and
moving operations will be provided within the total cost
under ‘Pay Item 1’ of the fee structure, per the MCDOT
barricade manual.
• Physical condition of the site.
• Safety of CONSULTANT’S personnel at the site and all
other persons or vehicles entering the site or areas
adjacent to the site; and,
• Security of CONSULTANT’S equipment at the site.
1.8 CONSULTANT shall be prepared at all times, at the express written
request of the COUNTY, to meet with the individual utility companies
and/or owner agencies to discuss work performed under this
Contract.
1.9 After written authorization to proceed via an assignment,
CONSULTANT shall perform the following professional services:

perform services specified in the assignment.
1.9.2 Secure and pay for all required permits.
1.9.3 Permits for work within jurisdiction of the COUNTY will be
issued at no charge to CONSULTANT.
2. Above Ground Utility Designating Services
2.1 The CONSULTANT shall obtain from the utility companies their
installation records for Aerial or Ground-Mounted facilities.
2.2 The CONSULTANT shall use visual observations to verify the
locations of the Aerial or Ground- Mounted utility facilities.
2.3 Quality level D or C services shall include records research,
identification, surveying, correlation, and/or depiction of aerial or

by blue-stake/AZ811 will not be considered payable,
unless authorized by the Utility Project Manager.
8.1.3 When measurements are completed and the utility is
properly identified, CONSULTANT shall backfill and
compact the test hole in compliance with the trenching,
bedding and backfilling requirements of the respective
utility and the governmental authority issuing the
excavation permit.
8.1.4 When the test hole location is in existing pavement,
CONSULTANT shall straight cut and remove the existing
pavement. After the utility is located, identified and the test
hole backfilled and compacted, CONSULTANT shall
restore the pavement to match the surface, materials, and
thickness at 1.25 times the existing pavement structure (per
Docusign Envelope ID: 87BDC578-5992-83A7-83BB-84B8D00F4B5B

roadway pavement, the backfill shall be as noted in
paragraph 8.1.3 above, and the disturbed areas shall be
restored, as neatly as possible, to the condition that
existed prior to the excavation.
8.1.8 The below (and enclosed) three (3) products are
authorized for restoration of small pavement utility cuts
including backfilling pavement cores/bore holes typically
up to approximately 6 inches in diameter. The products
are:
• Perma-Patch: Pressure/compaction activated, no
mixing or tack coat required, with a two-year
shelf life.
• Aquaphalt: Water-activated and compacted;
MCDOT has already had good experience with
this product.
• EZ Street: Polymer-modified cold asphalt
requiring no mixing, heat, or tack coat and
installed through compaction in lifts.
The use of the products are limited to small, isolated
utility cuts rather than continuous trench restoration or
larger pavement repairs. Proper backfill/base
preparation and compaction would still be required,
along with installation of the repair material in
accordance with the manufacturer's recommended lift
thickness and compaction procedures.
The table below shall be followed when determining if
the three products can be used in lieu of Conventional
HMA pavement restoration.
