Hi Walter,
I’ve been talking to the vendor as they’re asking how to go about requesting a replacement check. | sent them the
instructions and Affidavit/Notice of Claim that you sent me from before where we had the same issue.
It appears that their remittance address is correct but they never got the check in the mail. We have another invoice
from them that needs processed. They’re asking if it is possible to pick up the next payment to ensure that they get it.
I'll have someone from UCC pickup the check when it’s ready.

Got some bad news.
| looked in the excel log, file cabinet and the check never came back.
| can email them an affidavit for lost checks.
Is this email address correct :
Please confirm before | create the affidavit for the vendor.
For safety reasons, the vendor is not allowed to pick up the checks.
Your dept runner can pick it up or it can be mailed out after the BOS approve the
re-issue of check per affidavit. .
