I. The above-named contract is hereby amended as specified below:
A. Contract Expiration date shall be changed to June 30, 2022.
B. Attachment A, Pricing Sheet, TOTAL BUDGET is changed to $1,436,825. The
additional $1,058,825 is for the new contract period beginning July 1, 2021.
C. There are no changes in the Scope of Work.
II. All other terms and conditions of the original contract shall remain in full force and
effect.
