AMENDMENT 1 TO RFP CONTRACT 200002 WITH COLLABORATIVE RESEARCH FOR RYAN WHITE PART A PROGRAM CONSULTING SERVICES The above contract is hereby amended as specified below: • Additional funds in the amount of $650,000 for the period ending November 30, 2022 for a cumulative total of $1,400,000. The Scope for Work is amended to include: • Collaborative Research (CR) will provide technical assistance to support the Maricopa County Department of Public Health (MCDPH) in its efforts to Ending the HIV Epidemic Initiative activities in Maricopa County. Activities include but are not limited to: • Modernization and distribution of an HIV Resource Guide. • Assisting in the development and coordination of an Undetectable=Untransmittable (U=U) Media Initiative. • Design of a 24-Hour Live Response System for HIV Referrals and Linkages to Care. • Program-to-Client and Peer-to-Peer communication system. • Identification and development of systems level improvements and innovations. All other terms and conditions of the original contract shall remain in full force and effect.
