EXTENSION TO RFP CONTRACT 15039

DEPARTMENT OF PUBLIC HEALTH
And SOUTHWEST CENTER FOR HIV/AIDS
I. The above contract is hereby amended as specified below:
A. An extension from 5/31/2021 through 8/30/2021 to allow time for the successful
transition of all newly HIV diagnosed individuals from the Southwest Center for
HIV/AIDS Early Intervention Services program to other Ryan White Part A
contracted services.
B. The final invoice is due to MCDPH on or before September 15, 2021.
All other terms and conditions of the original contract shall remain in full force and effect.
