The Treasurer needs a statement that the person signing the affidavit 1) never received the check and 2) is an authorized agent to sign such a statement. This second requirement generally applies when we're dealing with a company, but in this case, as you are you, you are the authorized agent to sign for yourself. If you would. please return an email to me with those two articles of attestation, | can move forward with asking the Treasurer to cut a new check to you.
