CONTRACT NO: C86-21-063-X-01 AMENDMENT NO: 1 AMENDMENT TO PARTNERSHIP BETWEEN MARICOPA COUNTY DEPARTMENT OF PUBLIC HEALTH And PARTNER SCHOOL DISTRICTS/ORGANIZATIONS FOR THE IMMUNIZATION PROGRAM I. This PA is modified to include all age appropriate Childhood Vaccines and upon availability, Vaccines for Adults (VFA) to accompanying parents/guardians or staff who are uninsured or underinsured, to include Flu and COVID -19 vaccine (COVID-19 vaccinations to be based on qualifying age regardless of insurance status). II. The above-named amendment applies to the following Partners already under agreement: 1. Scottsdale Unified 2. Alhambra Elementary 3. Cartwright Elementary 4. Dysart Unified 5. Saddle Mountain Unified 6. Cave Creek Unified 7. Peoria Unified 8. Deer Valley Unified 9. Ombudsman Charter III. All other terms and conditions of the original contract shall remain in full force and effect.
