The Independent Contract Agreement dated July 1, 2020, between Arizona Community Action
Association DBA Wildfire and Maricopa County, administered by the Human Services
Department, Housing and Community Development Division (HCD) (hereinafter
“Contractor’) to conduct application intake and eligibility determination for utility bill assistance,
deposits and/or gas appliance repair/replace is hereby amended as follows:
Purpose of the Amendment:
1. To increase the Utility Repair Replacement Deposit (referred to as URRD) funds by the
amount of $20,000.00.
2. To revise Section 3 to add Paragraph 3.9.
Amendment to:
Section I. Services and Programs — 1.2 Fund Sources; Attachment 1:
Utility Repair Replacement Deposit (URRD): To increase the Direct Service amount to
$336,091.00 and increase the Program Delivery amount to $33,609.00. The new allocation
for URRD will be $369,700.00 (Direct Service + Program Delivery).
Section 3. Funding; Expenses; Nature of Relationship:
3.9 Written Certification Under A.R.S. § 35-393.01
If the Contractor engages in for-profit activity and has ten (10) or more employees and, if
this Agreement has a value of $100,000 or more, then the Contractor certifies it is not
currently engaged in, and agrees for the duration of this Agreement not to engage in, a
boycott of goods and services from Israel. This certification does not apply to a boycott
prohibited by 50 U.S.C § 4842 or a regulation issued pursuant to 50 U.S.C § 4842.
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement and Amendment
No. One (1) will remain in place for the duration of the original Agreement.

IN WITNESS WHEREOF, Amendment No. Two (2) shall be effective upon signature of the
Executive Director of Wildfire. The period of funding availability for this Amendment is July 1,
2020 through June 30, 2021.
