SECTION I. ACTION
The following change(s) will become effective when all approvals/signatures are completed. The Agreement is:
[X] Amended to include the following changes:
Public Official: Maricopa County administered by its Human Services Department.
Agent: Arbor E&T, LLC d/b/a ResCare Workforce Services – Title I - Youth program administered by the United States
Department of Labor (USDOL), Workforce Innovation Opportunity Act (WIOA) program services.
Name change: Arbor E&T, LLC. dba ResCare Workforce Services ("RWS”) has changed their name to Arbor E&T,
LLC. dba Equus Workforce Solutions (“EWS”).
1) Amend UIA-1000A, Page 4, No. 2 and Attachment A. (See amendment 175546 (Maricopa County No. C-22-18-013-3-02)
Change Agent/Partner name to Arbor E&T, LLC. dba Equus Workforce Solutions (“EWS”).
Except as specified above, all the terms of the original Agreement will continue to be in effect.
