Amendment No. Two (2) to the 2019-2020
Independent Contractor Agreement
Contract No. 07012019-20
The Independent Contract Agreement dated July 1, 2019, between Arizona Community Action
Association DBA Wildfire and Maricopa County – Human Services (hereinafter “Contractor”)
to conduct application intake and eligibility determination for utility bill assistance, deposits and/or
gas appliance repair/replace is hereby amended as follows:
Purpose of the Amendment:
1. To increase the Arizona Public Service Crisis Bill Assistance Program (referred to as APS
BA) funds by the amount of $378,718.00.
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources; Attachment 1:
Arizona Public Service Crisis Bill Assistance Program (APS BA): To increase the Direct
Service amount to $367,908.00 and increase the Program Delivery amount to $73,582.00.
The new allocation for APS BA will be $441,490.00 (Direct Service + Program Delivery).
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in place
for the duration of the original Agreement.
