AMENDMENT NO. 2
TO
AGREEMENT BETWEEN
MARICOPA COUNTY, BY AND THROUGH MARICOPA COUNTY ATTORNEY’S OFFICE,
AND SCOTTSDALE HEALTHCARE
HOSPITALS d.b.a. HONORHEALTH
This Amendment No. 2 (the “Amendment”) to the Agreement between Maricopa County,
by and through Maricopa County Attorney’s Office and Scottsdale Healthcare Hospitals
d.b.a. HonorHealth (the “Agreement”) involves changes within Section 1.3 ROLES &
RESPONSIBILITIES of the Agreement to include adding Chandler Family Advocacy
Center as a common site for FNE response.
The parties hereto now desire to amend the terms of the Agreement as follows:
1. Under section 1.3.2 (“The most common sites that FNE respond to are as
follows:”) of the Agreement, the following location shall be added to the list as
a new 1.3.2.6:
Chandler Family Advocacy Center
221 East Boston Street
Chandler, AZ 85225
Added nurse response time of 15 minutes may be allowed for the Chandler
Family Advocacy Center location.
Both parties agree that all other clauses in the Agreement except as amended herein
shall remain in full force and effect. If there is a conflict between this Amendment and the
Agreement, the terms of this Amendment will prevail. All references to the Agreement
shall be deemed to mean the Agreement as modified hereby.
