Amendment No. One (1) to the 2019-2020
Independent Contractor Agreement
Contract No. 07012019-20
The Independent Contract Agreement dated July 1, 2019, between Arizona Community Action
Association DBA Wildfire and Maricopa County – Human Services (hereinafter “Contractor”)
to conduct application intake and eligibility determination for utility bill assistance, deposits and/or
gas appliance repair/replace is hereby amended as follows:
Purpose of the Amendment:
1. To increase the Arizona Public Service Crisis Bill Assistance funds by the amount of
$17,509.00.
2. To increase the Salt River Project Bill Assistance funds by the amount of $1,540.00.
3. To increase the Southwest Gas Energy Share Bill Assistance funds by the amount of
$30.00.
4. To increase the Utility Repair Replacement Deposit funds by the amount of $2,420.00.
5. To remove expenditure limit of Arizona Public Service Crisis Bill Assistance for FY20.
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources:
Arizona Public Service Crisis Bill Assistance: To increase the Direct Services amount to
$52,310.00 and increase the Program Delivery amount to $10,462.00. The new allocation
for Arizona Public Service Crisis Bill Assistance will be $62,772.00 (Direct Service +
Program Delivery).
Salt River Project Bill Assistance: To increase the Direct Services amount to $113,259.00
and increase the Program Delivery amount to $11,326.00. The new allocation for Salt
River Project Bill Assistance will be $124,585.00 (Direct Service + Program Delivery).
Southwest Gas Energy Share Bill Assistance: To increase the Direct Services amount to
$32,209.00 and increase the Program Delivery amount to $3,221.00. The new allocation
for Southwest Gas Energy Share Bill Assistance will be $35,430.00 (Direct Service +
Program Delivery).
Utility Repair Replacement Deposit: To increase the Direct Services amount to $57,091.00
and increase the Program Delivery amount to $5,709.00. The new allocation for Southwest
Gas Energy Share Bill Assistance will be $62,800.00 (Direct Service + Program Delivery).

Section I. Services and Programs – 1.2 Fund Sources; Attachment 1:
Arizona Public Service Crisis Bill Assistance: Contractor can spend the Direct Service
amount of $52,310.00 and the Program Delivery amount of $10,462.00 for a total of
$62,772.00 (Direct Service + Program Delivery) by June 30, 2020 however they deem
appropriate.
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in place
for the duration of the original Agreement.
