Corte Bella During a duly noticed meeting held on December 21, 2017, the Governing Board of the North
County Fire & Medical District voted unanimously to accept the annexation of the property
Cross River more particularly described as set forth in the attached Resolutions #17-1221-1. Per A.R.S.
Dos Rios §48-262, the areas being annexed are contiguous to the current Fire District boundaries.

Board Chairman Wilson called the meeting to order on Thursday, December 21, 2017, at 1:30
p.m. at the Maricopa County Library, Ed Robson Branch Leky Community Room, 9330 E.
Riggs Road, Sun Lakes, Arizona 85248.

Board Member Cheney moved that the Governing Board approve all items on the
Consent Agenda, including the November 2017 Monthly Financial Reports and
account reconciliations per Arizona Revised Statutes including, but not limited to,
the following: Register of Checks, Warrants, & Deposits; Statement of Financial
Activities; Statement of Net Assets; and Cash FlOW Projection Report. Board
Member Miller seconded the motion.

7. DISCUSSION & POSSIBLE ACTION RE: PROPERTY OWNER ANNEXATION REQUEST:
RESOLUTION NO. 17-1221-1 — AUTHORIZING ANNEXATION REQUEST — PARCEL
#201-17-006N, LOCATED AT 26222 N. 102n d AVENUE, MARICOPA COUNTY
Assistant Chief Dalton presented information to the Board Members related to the annexation
request and answered any questions from the Board Members.
Board Member Miller moved that, following Board discussion which determined that
District annexation of the proposed property would benefit both the property owner and
the Authority, that the Governing Board adopt Resolution #17-1221-1 authorizing the
annexation request of the property owners for the property as described in Exhibits A and
B attached to the resolution. Board Clerk Cherry seconded the motion. Discussion
followed.
Board Member Miller amended her motion to move that, following Board discussion which
determined that District annexation of the proposed property would benefit both the
property owner, the Arizona Fire & Medical Authority, and the North County Fire &
Medical District, that the Governing Board adopt Resolution #17-1221-1 authorizing the
annexation request of the property owners for the property as described in Exhibits A and
B attached to the resolution. Board Clerk Cherry seconded the motion.

A list of suggested items will be discussed during the Arizona Fire & Medical Authority
immediately following this meeting.

BOARD OF DIRECTORS RESOLUTION NO. 17-1221-1
A FORMAL RESOLUTION OF THE DETERMINATION OF THE ELECTED BOARD OF
NORTH COUNTY FIRE & MEDICAL DISTRICT ORDERING THAT THE DISTRICT
BOUNDARIES BE CHANGED TO INCLUDE THAT PROPERTY (PARCEL #201-17-006N)
DESCRIBED IN EXHIBIT "A" ATTACHED HERETO:
WHEREAS, the North County Fire & Medical District Board of Directors has been presented with
a request for inclusion of the property described in Exhibit "A" attached hereto, requesting that
said property be included into the jurisdictional boundaries of the District; and
WHEREAS, the subject property is located within Maricopa County and is contiguous to the
boundaries of North County Fire & Medical District as shown on Exhibit "B" attached hereto; and
WHEREAS, all requirements of A.R.S. §48-262 have been addressed and met according to law;
and
WHEREAS, the North County Fire & Medical District Governing Board has determined that the
inclusion of the subject property within the boundaries of North County Fire & Medical District
will benefit North County Fire & Medical District and the property owners.
NOW, THEREFORE, BE IT RESOLVED that the Governing Board of North County Fire &
Medical District does hereby find that the proposed inclusion of the property satisfies the
requirements of A.R.S. §48-262, and does hereby order the change to the District boundaries to
include the property as set forth in the attached legal description Exhibit "A" and map Exhibit "B"
in North County Fire & Medical District.
RESOLVED AND ADOPTED at a duly noticed public meeting of the Governing Board of the
North County Fire & Medical District on December 21, 2017, by a majority of the Governing
Board Members.
