AMENDMENT TO RFP CONTRACT BETWEEN
MARICOPA COUNTY by and through its
DEPARTMENT OF PUBLIC HEALTH
AND FALCON PRACTICE MANAGEMENT, LLC
I. The above named contract is hereby amended as specified below:
1.0 Contract Term:
1.1 This Contract is for a term of three (3) months, beginning on the 8th day
of September, 2019 and ending the 7th day of December, 2019.
II. The BAA remains effective and a part of this contract.
III. All other terms and conditions of the original contract and all amendments shall
remain in full force and effect.
