Please find attached 1 Affidavit for a forged warrant. The name on the warrant was
changed and cashed by an unknown person. Please include the warrant on the next
available Board Agenda.
• The attached items have been researched and investigate to confirm that
the warrant in question was indeed forged and cashed by and unknown
person. Chase bank investigated the forgery and confirmed it occurred.
The funds are returned to Maricopa County Dept. or Finance.
Please call me at 506-1343 if you have any questions. Please return the approved
Affidavits to my attention in the Department of Finance, Suite 960.

MAKER PLEASE DESCRIBE:
On July 8th, 2019 Krystal in the Dept. of Finance was contact by the S. Marie Looney inquiring about a check she
hadn't received in the mail. Check #3010036386 was issued on June 17,2019 and cleared the bank on 6/28/19.
The name and address on the warrant are altered. The Dept. of Finance has a copy of the Cleared Warrant
with the altered payee name

On July 8th, 2019 I contact [Crystal at Madcap County Department of Finance inquiring about a check I hadn't receive.
I was informed that the check #3010036386 for $4062.00 was cashed and the name on the check was altered.
I also stated that I don't know the anyone by the name Melvy Yaraly Garate Esqu. I'm a contract employee for
OrMaricopa County. I'm paid the 20th every month. My checks are received by mail. I did not receive my
check and was told to wait (see attached emails) I folfowed up on 7/8/19 instead of waiting until 7110119. The
Finance Dept, for the county emailed me a copy of my check being forged by someone else.
