C-22-17-010-3-04/G-30200 Amendment No. 3 AMENDMENT No. 3 TO INTERGOVERNMENTAL AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND MESA UNIFIED SCHOOL DISTRICT I. Maricopa County, by and through the Maricopa County Human Services Department (“County”), and Mesa Unified School District (“District”) entered into an Intergovernmental Agreement on or about November 25, 2016, as amended on July 13, 2017 by Amendment No. 1 and amended on March 14, 2018, by Amendment No. 2 (collectively, “Agreement”). The Agreement term began August 1, 2016 through June 30, 2019. The Agreement contains an option to extend the Agreement for two additional 3-year terms, upon written approval by the Parties. The District and the County are collectively referred to herein as the “Parties” and individually as a “Party.” The purpose of the Agreement is to establish a process in which services are provided to preschool children, including children with disabilities as defined in A.R.S. §15-761, ages three years old to kindergarten eligible five-year-olds, in compliance with federal and State laws and regulations, and in accordance with Mesa Unified School District policy, and Improving Head Start for School Readiness Act of 2007 (42 U.S.C. §§ 9801, et seq.), by referring eligible children to the County’s Head Start program and the Head Start program shall refer eligible children to the District. II. The Parties agree to enter into this Amendment No. 3 to the Agreement (“Amendment”) to amend the Agreement as follows, (Amendment No. 1, renewal option #1): A. Extend the Agreement term beginning retroactively as of July 1, 2019 to June 30, 2020. B. Revise Facility Use Section of the Agreement, Paragraph A to extend Head Start program service delivery to include the following District’s school locations: Adams Elementary School, 738 S. Longmore 85202 Edison Elementary School, 545 N. Horne 85203 Hughes Elementary School, 630 N. Hunt Drive 85203 Lindbergh Elementary School, 930 S. Lazona 85204 Lincoln Elementary School, 930 S. Sirrine 85210 Longfellow Elementary School, 345 S. Hall 85204 Lowell Elementary School, 920 E Broadway 85204 Red Mountain Center, 950 N Sunvalley Blvd. 85207 Stevenson Elementary School, 638 S. 96th Street Taft Elementary School, 9800 East Quarterline 85207 85208 Webster Elementary School, 202 N. Sycamore 85201 Whitman Elementary School, 1829 N.
