AMENDMENT TO INTERGOVERNMENTAL AGREEMENT (IGA)
Between MARICOPA COUNTY by and through
DEPARTMENT OF PUBLIC HEALTH
And the ARIZONA BOARD OF REGENTS for
and on behalf of the ARIZONA STATE UNIVERSITY
I. Per Section I, 22., Right to Extend Agreement, the above named IGA is hereby amended
retroactively as specified below to extend the Agreement period and provide funds not-to-
exceed $9,000.00 for the period of July 1, 2019 through June 30, 2020:

3. Agreement Amount: $9,000.00 (3 year cumulative amount $33,000)
6. Expiration Date: June 30, 2020
B. Page 13, Section III. 3. A revise in its entirety to read:
A. Tobacco Use Prevention Materials and/or Coordinator Stipend(s): with satisfactory
completion of the stated objectives, Arizona State University shall receive a $9,000 stipend
for its four campuses (Downtown: Phoenix, main: Tempe, Poly and West). Funds may be used
to purchase materials/services in support of the IGNITE (Influence, Guide, Network, for
Intercollegiate Tobacco Education) County's program requirements and/or be used as
coordinator stipend(s).
C. Page 14, Section III. 3. B, replace the word “Incentive” with the word “Promotional”
D. Page 14, Section III. 4. revise in its entirety to read:
4. SERVICE OBJECTIVES/ACTIVITIES
The Contractor agrees to provide the following required services:
A. Complete at least 2 projects focused on promoting tobacco prevention and/or cessation
and/or raising awareness about or promote adoption, implementation, or enforcement
of strong tobacco-free policy on campus.
B. All required reports and evaluations will be submitted by the 1st of each month for
activity completed in the prior month by electronic form to assigned OSHWI Prevention
Specialist, link to electronic form will be provided during coordinator orientation and via
monthly reminders to coordinators.
C. Activity reports are required to be submitted to the designated OSHWI staff in order for
invoices to be processed.
*** Please note that any variance from the above stated activities must be approved by
your OSHWI IGNITE Coordinator.

E. Page 16, Section IV. 1 ., revise to read in its entirety:
1. COMPENSATION
Subject to the availability of funds, County shall pay the Contractor for the
services described herein for a sum not-to-exceed the Contract Amount listed on
the cover page of this contract. The Contractor may request reimbursement up
to 2 times during the contract year. The first invoice may include any completed
project as identified in SECTION Ill. 3A. The second invoice may include any
additional completed project identified in SECTION Ill. 3A. The Contractor may
choose to submit a single invoice for all completed projects.
F. Replace Attachment A with the revised Attachment A attached to this Amendment No. 3 as
page 3.
II. All other terms and conditions of the original contract and its amendments shall remain in full
force and effect.
