The Arizona Governor's Office of Highway Safety (GOHS) is pleased to inform you that your application
for funding has received final approval in the amount of $20,000.00 to support the following: Vehicular
Heatstroke Campaign. This includes the May and August advertising: seven outdoor digital billboards,
80 radio spots, earned broadcast along with print media and some digital social media ads as your media
plan of May 20, 2019.
Funding allocations are made on a reimbursement basis only. Once your project has been completed,
please send your supporting documentation to:

Please include the following:
• Invoices, receipts, or paid bills
• Evidence that the project activity took place and was completed (i.e., Progress report)
Neglecting to include these items may delay your payment. Reimbursement processing will take place
once all documentation is received. If you need additional time to complete your project, your request for
an extension must be made in writing. explaining the need for additional time.
