AMENDMENT NO. 3
TO THE
SUBRECIPIENT AGREEMENT
BETWEEN MARICOPA COUNTY
ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT
AND
COMMUNITY BRIDGES, INC.
I. Maricopa County (“County”) by and through its Human Services Department and Community
Bridges, Inc. (referred to as “Subrecipient”) entered into a Subrecipient Agreement
(“Agreement”) on or about September 14, 2017, for the provision of rapid rehousing services
for homeless persons in Maricopa County. The County provided the Subrecipient with
$92,000 PY2017 Emergency Solutions Grant (ESG) funds and $106,000 PY2017 Community
Development Block Grant (CDBG) funds for these services. The Agreement term began on July
1, 2017 to June 30, 2018. The County and the Subrecipient may be referred to individually as
the “Party” or collectively as the “Parties.”
The Parties executed Amendment No. 1 on or about March 23, 2018 to correct an
administrative error. The Parties executed Amendment No. 2 on or about June 28, 2018 to
add an additional $92,000 in PY2018 ESG funds and $106,000 in PY2018 CDBG funds to
continue rapid rehousing services. The Amendment also extended the tem of the Agreement
from June 30, 2018 to June 30, 2019.
II. The Parties now wish to amend the Agreement as follows (Amendment No. 3):
A. Extend the Agreement termination date from June 30, 2019 to June 30, 2020.
B. The County shall provide Subrecipient with $103,000 in PY2019 CDBG funds and
$72,949 in PY2019 ESG funds. Amendment No. 3 total funds shall not exceed
$175,949 for continued rapid rehousing services for homeless persons in Maricopa
County.
C. Period of funding availability shall begin on July 1, 2019 through June 30, 2020.
D. Revise Section III, Work Statement to add to paragraph A, subparagraph 7:
Subrecipient shall submit the final reimbursement request for the fiscal year to
Maricopa County no later than July 15.
III. The County shall reimburse the Subrecipient on a Net 0 payment standard.
IV. This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs contain all the changes
made by this Amendment. All other terms and conditions of the original Agreement remain
the same and in full force and effect as approved and amended.
