AMENDMENT NO. 1 TO THE
CONTRACT
BETWEEN MARICOPA COUNTY
ADMINISTERED BY ITS HUMAN SERVICES
DEPARTMENT
AND
HUMAN SERVICES CAMPUS, INC.
Maricopa County (“County”) by and through the Maricopa County Human Services Department and
Human Services Campus, Inc. (referred to as “Contractor”) entered into a Contract on or about January 1,
2019, for the provision of emergency weather relief accommodations for individuals experiencing
homelessness at the Human Services Campus. The County provided the Subrecipient with $3,042.12 in
FY19 Maricopa County General Funds for these services. The County and the Subrecipient may be referred
to individually as “Party” or collectively as “Parties.”
The Parties now wish to amend the Agreement as follows:
I. Extend the contract termination date from June 30, 2019 to June 30, 2020.
II. This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs, contain all the changes
made by this Amendment. All other terms and conditions of the original agreement remain the
same and in full force and effect as approved and amended.
