AMENDMENT
TO THE AGREEMENT
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
HUMAN SERVICES DEPARTMENT
AND
TEMPE COMMUNITY ACTION AGENCY
Maricopa County by and through its Human Services Department (“County”) and Tempe Community
Action Agency (TCAA), (“Contractor”) entered into a Contract Agreement (“Agreement”) on or about
August 23, 2017. The Agreement term began on July 1, 2017 through June 30, 2018 and has two additional 1-
year terms. The County and TCAA may be referred to individually as “Party” or collectively as “Parties.”
The Parties executed Amendment No. 1 which increased the reimbursement amount by $300,000 from
$615,000 to $915,000 for the July 1, 2017 to June 30, 2018 term. The Parties executed Amendment No. 2,
which extended the agreement term from June 30, 2018 to June 30, 2019, renewal option #1. The Parties
executed Amendment No. 3, which increased the reimbursement amount by $500,000, from $900,000 to
$1,400,000 and increased the check writing fees by $10,000, from $15,000 to $25,000 for the July 1, 2018 to
June 30, 2019 term.
The purpose of the Agreement is for TCAA to make payments to landlords, mortgage companies or utilities
companies on behalf of eligible clients that require financial assistance.
The Parties wish to amend the Agreement to address the following areas (Amendment No. 4, renewal option
#2):
I. Extend the Agreement term from June 30, 2019 to June 30, 2020 pursuant to the terms of the
Agreement.
II. Amendment No. 4 shall be effective on July 1, 2019 through June 30, 2020.
A. The funding amount for reimbursements to be paid to landlords, mortgage and utilities shall
not-exceed $1,425,000 for the period of July 1, 2019 to June 30, 2020.
B. The check writing fees shall remain at a not-to-exceed amount of $25,000.
III. The Contractor shall be reimbursed at a Net “0” payment standard.
IV. The Amendment shall be effective upon approval and signature by both parties.
V. The foregoing paragraphs contain all the changes to the Agreement. All other terms and conditions
of the original Agreement as amended and approved remain the in full force and effect and as agreed
to by the Parties.
