I. The above named contract is hereby amended as specified below:
A. Section 1.0 Contract Term, 1.1, revised to read: ending the 31st day of
December, 2019;
B. Exhibit A, Compensation/Fees: revised to read Public Health
Disease/Emergency Reporting Lines (3) $75,000 per year
II. All other terms and conditions of the original contract and any previous amendments
shall remain in full force and effect.
