AMENDMENT TO STUDENT ROTATION TRAINING AGREEMENT
BETWEEN
MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
THE ARIZONA BOARD OF REGENTS, acting for and on behalf of
ARIZONA STATE UNIVERSITY
The above named agreement is hereby amended as specified below.
A. This Off-Site preceptor Student Rotation Training Agreement ("Agreement") is
entered into by and between the Arizona Board of Regents, acting for and on
behalf of Arizona State University ("ASU"), and Maricopa County, by and for its
Departments of Public Health and Correctional Health ("MCDPH"). This
Agreement sets forth the terms of the rotation of Students from ASU to MCDPH
for educational training and experience. This Agreement shall commence on
July 1,2016, and is valid through June 30, 2021."
All other terms and conditions of the original Agreement shall remain in full force and
effect.
IN WITNESS WHEREOF, the parties agree to the changes indicated herein:
