formerly known as
DIGNITY HEALTH, a California nonprofit public benefit corporation
d/b/a ST. JOSEPH’S HOSPITAL AND MEDICAL CENTER
I. The above named contract is hereby amended as specified below:
A. This amendment to the Adjunct Affiliation Agreement for Medical Education will
recognize a name change to the “Sponsoring Organization” from DIGNITY
HEALTH, a California nonprofit public benefit corporation d/b/a ST. JOSEPH’S
HOSPITAL AND MEDICAL CENTER (SJHMC) to DIGNITY COMMUNITY
CARE, a California nonprofit corporation, d/b/a SJHMC.
II. All other terms and conditions of the original contract shall remain in full force and
effect.
