AMENDMENT TO STUDENT ROTATION TRAINING AGREEMENT
BETWEEN
MARICOPA COUNTY, by and through its DEPARTMENT OF PUBLIC HEALTH
AND
CAPELLA UNIVERSITY
I. The above named agreement is hereby amended as specified below.
A. The original contract shall be amended to change the opening paragraph of the
contract to: This Student Rotation Training Agreement ("Agreement") sets forth
the terms of the rotation of Students from Capella University ("CAPELLA"), a
Minnesota corporation whose principal office is located at 225 S. 6th St•,
Minneapolis, MN 55402 to Maricopa County ("MCDPH"), by and for its
Department of Public Health, whose principal office is located at 4041 N, Central
Ave., Ste. 1400, Phoenix, AZ 85012, and its Department of Correctional Health
Services, whose principal office is located at 3250 W. Lower Buckeye Rd., Ste.
2100, Phoenix, AZ 85009 for educational training and experience. This
Agreement shall commence on February 1, 2018, and is valid through June 30,
2022,"
All other terms and conditions of the original Agreement shall remain in full force and
effect.
