SALT RIVER PIMA-MARICOPA INDIAN COMMUNITY 10005 E. Osborn Road, Scottsdale, Arizona 85256 / Phone 480-362-7520 / 12% Gaming Contributions Application Date of Application: July 18, 2018 Organization Name: Arizona Heart Foundation Contact Person: Paula Banahan, RN Email Address: pbanahan@azheartfoundation.org Phone: 602-707-3531 Organization Mailing Address: 1910 E Thomas Rd., Ste #100, Phoenix, AZ 85016 Project Name: AHF Cardiovascular & Diabetes Community Initiative Grant Award Request: $50,000 Please provide a brief description of the project being submitted to the 12% SRPMIC Grant program: Diabetes is a major epidemic in our country with annual, associated medical costs estimated at more than $245 billion a year. But the cost is not only financial. Diabetes affects all areas of the body – impaired sight and blindness, hearing loss, peripheral nerve damage (which can lead to amputations), kidney damage, and cardiovascular disease. At least one in three adults is “prediabetic,” yet most – about nine out of ten – do not even know it. Prediabetes is not a “safe” condition as cardiovascular damage is already occurring within the blood vessels. Adults with diabetes have two to four times higher rates of death from heart disease or stroke. Education is the key to prevention. The Arizona Heart Foundation has developed a public information program to educate participants on how a change in lifestyle, to include an appropriate diet and increased activity, can greatly lessen or even eliminate the chance of developing Type-2 Diabetes. Unfortunately, poor diets, combined with a lack of physical exercise, have led to a major obesity crisis in the U.S., which is a precursor to Diabetes. Our target demographic is adults between the ages of 18-44 – i.e., those individuals who have the greatest potential of successfully slowing the progression or even avoiding the disease. After learning about diabetes and its manifestations, this age group also has the greatest potential of sharing the knowledge acquired to influence their parents, siblings, and peers to adopt these same lifestyle changes. Weekly programs will be offered at the Arizona Heart Foundation and presented by subject matter experts. During a 16-week period, information on nutrition will be provided on everything from how to shop for, prepare, and include healthy food choices in your everyday life. Tips will be shared on how to avoid temptations to go “off course”, behavior strategies as well as encouragement and tools to manage the stress and complications brought on by this change. Guidance will also be given on exercise (from easy movement activities through full workouts) that must be included for a healthy lifestyle. Over a 12-month period, goals will include losing a minimum of 5% of total body weight, reducing waistline, and reducing body mass index (a measurement of body fat or BMI) to a healthy number. Quarterly assessments will be conducted to monitor progress. The Arizona Heart Foundation program will serve the greater Phoenix area by offering a scientifically proven and effective, year-long lifestyle change, pilot program that can help prevent or delay cardiovascular disease, insulin-resistance, and/or Type-2 Diabetes. This program can also be customized and/or conducted at other locations throughout Maricopa County as needed.
