Clarendon Ave, Suite 200, Phoenix, AZ 85013 Project Name: Expanded Medical Respite Care for Homeless Persons in Maricopa County Grant Award Request: $50,000 Please provide a brief description of the project being submitted to the 12% SRPMIC Grant program. Founded by Sister Adele O'Sullivan, CSJ, MD, in 2008, Circle the City's (CTC) mission is to create and deliver innovative healthcare solutions that compassionately address the needs of men, women and children facing homelessness. CTC is expanding its medical respite care program for medically vulnerable patients who are homeless by renovating a 15,000square foot building on the Human Services Campus in downtown Phoenix. The new medical respite care center will have 50 beds. The current medical respite care site in central Phoenix also has 50 beds and a 30 to 50 patient waiting list at any given time—as the need for medical respite beds is constant and ongoing. The new site on the Human Services Campus will bring the number of medical respite beds for medically vulnerable persons who are homeless to a total of 100 and will ease the waiting list. CTC's Medical Respite Center has quickly become one of the nation's leading models for holistic, integrated healthcare for patients who are homeless. Respite patients will meet daily with compassionate physicians and nurses to ensure that their medical needs are addressed, including chronic disease management and maintenance of geriatric conditions such as cognitive impairment and frailty. Along with primary health supports additional on-site services include: pain management, medication management, psychiatric evaluation and treatment, substance abuse intervention, occupational therapy, and physical therapy. Patients will also work closely and regularly with case managers to apply for necessary documentation (identity, income,); benefits including income, health insurance; and housing. These measures are taken to ensure that patients are successfully discharged to safe living environments. In fact, of CTC's patients who have completed their medical plans of care, 78% have been placed into housing. CTC Medical Respite Centers offer patients experiencing homelessness, regardless of their ability to pay, an opportunity to heal in an environment of respect and dignity as they journey toward healthy, independent living.
