SALT RIVER PIMA-MARICOPA INDIAN COMMUNITY 10005 E. Osborn Road, Scottsdale, Arizona 85256 / Phone 480-362-7520 12% Gaming Contributions Application Date of Application: July 24, 2018 Organization Name: Community Bridges, Inc. Contact Person: Gabriella Guerra, Chief Program Officer Email Address: gguerra@cbridges.com Phone: (480) 831-7566 Organization Mailing Address: 1855 West Baseline Road, Suite 101 Mesa, AZ 85202 Project Name: Temporary Shelter Bridge to Permanent Housing Grant Award Request: $64,496 Please provide a brief description of the project being submitted to the 12% SRPMIC Grant program. Community Bridges, Inc.'s (CBI) 34-bed temporary shelter will serve homeless adult males who have a permanent housing solution, have been referred through coordinated entry, and have a mobile case manager (or navigator) who will assist the participant through the lease up process to enter permanent housing. In most cases, a permanent housing solution will be a housing voucher, but it may include individuals with income who will rent on their own. CBI's Temporary Shelter Bridge to Permanent Housing Project is a collaboration between the City of Phoenix and Mercy Maricopa Integrated Care (MMIC). The purpose of the project is to reduce the length of time it takes for homeless singles to transition from homelessness to permanent housing once they have been referred for housing. CBI may be the long-term support service provider through one of its homeless housing programs; however, the temporary shelter will be available to all housing and outreach providers in Maricopa County. CBI's new temporary shelter facility is the result of a renovation of CBI's Central City Addiction and Recovery Center (CCARC), a 23,000 square foot single-story facility located in the southwest corner of the larger 86-acre Arizona State Hospital Campus. The temporary shelter is a renovation of approximately 3,600 sq. ft of the northeastern section of the dormitory-type facility. The newly renovated shelter includes a 34-bed sleeping quarters, showering area, recreation/seating area, group room, storage room, offices and central monitoring station. Seven fulltime equivalent Navigator II positions are required to staff the facility 24/7 with one Navigator at all times. The Shelter Navigator will review and verify the coordinated entry referral packet documentation before accepting the participant for shelter. The Navigators will maintain a case file for each participant and include the documents in the referral packet, case notes and updates to the participant's SPDAT and housing case plan. Navigator will review the full SPDAT assessment and case plan completed by the participant's housing case manager and meet with the housing case manager for case conferencing within 5 days or less of the participant's shelter entry. Shelter navigators will conduct weekly case conferencing with each participant's long-term housing case manager. The shelter staff and the long-term housing provider will progressively engage with the participant to develop and implement a transition strategy to permanent housing. In addition, the Shelter Navigator will enter case notes and new goals as needed based on shelter observations and case conferencing with the housing case manager. The shelter will have computers for participant use for benefit applications, job search, resume building, appointment scheduling, information research, etc. Shelter navigators can assist with application processes for benefits and will provide on-going life skills education groups. The Navigators will be facilitating groups like housing readiness, eviction prevention, and the Living in Balance curriculum. One fulltime Program Manager will work 40 hours per week to manage the facility including reviewing client admissions and discharges, ensuring compliance with program policies, creating program policies, reviewing performance data, and managing program staff.
